He is survived by: his parents, Kathy L. Jakubowski and Dave'Jake' Jakubowski (Yolanda); his son Hunter Jakubowski; and his brothers, Rich Jakubowski (Lori) and Paul Jakubowski (Rebecca). He is also survived by several aunts, uncles, nieces, nephews and cousins.
