She was predeceased by : her parents, Joe Mack and Helen Mack. She is survived by : her son Chad Perkins, Sr.; her daughter-in-law Kathryn Herzog (Jason); her grandchildren, Elizabeth, Chad, Jr. (Julianna) and Kyler; her great grandchild Alaia; her siblings, Dr. Daniel J. Mack, Sr. (Kim), Thomas Mack (Bebe), Catherine Wilson (Richard), Mary Darcangelo (Ron) and Sharon Cooper; and her friend Chris Homa. She is also survived by many nieces, nephews, aunts, uncles.
