He was predeceased by: his parents, Francis Cook and Theresa Ferrone. He is survived by: his wife Lynn (Madison); his brother Frank Cook of Newburgh; his children, Diana Peloza (John) of Irvine, CA, John Cook, Jr. (Martine) of North Brookfield, NY, Jeanne Cook (Dave Coates) of Montgomery, NY, Frank (Rebecca) of Cheyenne, WY and James (Jen) of Stamford, CT; and his grandchildren, Samantha, Michaela, Cora, Amelia, Eileen and Everett. He is also survived by numerous nieces, nephews, cousins, and friends.
Visitation was held on Tuesday, September 8th 2026 from 2:00 PM to 4:00 PM and from 6:00 PM to 8:00 PM at the Applebee-McPhillips Funeral Home (130 Highland Ave, Middletown, NY 10940). A graveside service was held on Wednesday, September 9th 2026 from 11:00 AM to 11:15 AM at the Woodlawn Cemetery (93 Union Ave, New Windsor, NY 12553).
In lieu of flowers, the family would like donations to Jack's favorite charities: Tunnel to Towers Foundation and Wounded Warrior Project.
