He was predeceased by: his parents, Justo Velazquez Nieves and Carmen Ana Velez Ramos; and his sister Carmen Pergolizzi. He is survived by: his wife Eneida Mary Velazquez; his children, Justin Velazquez (Sasha) of Longwood, FL and Nicole Rivera (Manuel) of Jersey City, NJ; his siblings, Ulysses Tellado of Phoenix, AZ, Alida Rivera of Staten Island, NY and Roseanna Tellado of Kissimmee, FL; and his grandchildren, Jacen, Zoe and Luke. He is also survived by many nieces and nephews.
