He was predeceased by: his parents, Edward W. Forman and Edna Forman (Zangenberg); and his daughter Karel. He is survived by: his wife Florence Koscielny; his children, Kurt of Palmyra, Keith (Elizabeth) of Troy and Dr. Kirsten Forman-Bailey (Ken Bailey) of Clifton Springs; his grandchildren, Luke Forman, Hope Forman, Eleanor Forman and Josephine Forman; and his great grandchild Autumn Forman.
