He was predeceased by: his parents, Joseph D'Agostino (late Lena) and Mary D'Agostino (Kendzia); and his sister Susan Costanzo. He is survived by: his wife Sharon D'Agostino (Pawlak); his children, Jacqueline D'Agostino, Frank D'Agostino, Jenna Lysiak (John) and Matthew D'Agostino; his former wife Kathleen Palazzo; his grandson Joseph Mangione; and his siblings, Joseph D'Agostino and Linda D'Agostino. He is also survived by his many cousins, nieces, nephews, and a wide circle of dear friends.
In lieu of flowers, in honor of Chic's compassionate spirit, the family kindly requests that memorial offerings be directed to an animal rescue organization of one's choosing.
