He was predeceased by: his parents, John E. Belkota and Theresa Belkota (Raymond). He is survived by: his sisters, Anita Stiglet (Sean), Julie Belkota, Theresa "Jo Jo" Belkota, Mari Beanblossom (Lacy) and Michelle Matyjas (Matt). He is also survived by several nieces and nephews.
