(Dudek) Forman of North Tonawanda, New York, born in Buffalo, New York, who passed away on June 3, 2025, at the age of 92, leaving to mourn family and friends. She was predeceased by: her parents, Alexander Dudek and Victoria Dudek; her husband Leon Forman; her siblings, Walter Dudek, Alexander Drew and O'Tillia Salzyn (Stan); and her great-grandparent Mimi.
She is survived by: her children, Dawn Kozak (Joe) and Leon Forman (Kathy); and her grandchildren, Jennifer Genovese (Joe), Sherri George (Bob), Joseph Kozak, Lee Forman and Scott Kozak. She is also survived by many nieces and nephews.
Visitation was held on Thursday, June 5th 2025 from 4:00 PM to 7:00 PM at the Wattengel Funeral Home (533 Meadow Dr, North Tonawanda, NY 14120). A Mass of Christian burial will be held on Friday, June 6th 2025 at 9:30 AM at the St. Christopher R. C. Church (2660 Niagara Falls Blvd, Tonawanda, NY 14150).
