He is survived by: his wife Bridget Fazio; his children, Schon Fazio of Sarasota, FL and Justin Fazio (Jessica) of South Plymouth, NY; his grandchildren, Jocelynn, Sophia and Kallix; his siblings, Thomas Fazio, Jr., Timothy Fazio (Jill) and Steven Fazio; and his nephew Brandon Fazio.
