He was predeceased by: his parents, Clifford Cook and Marjorie "Joyce" Cook; and his sister Linda White. He is survived by: his wife Susan Cook; his daughter Shannon Cook; his stepchildren, Rachel Pyle (Ben Sikora), Rebecca Whited (William) and Bradley Pyle (Michele); and his sisters, Juanita Taber (James) and Susan Colbert. He is also survived by many nieces and nephews.
