Honan (Katherine) and Kelly Carosa (Joe); her grandchildren, Cal, Chase, James and Adeline; and her siblings, Dawn Puglisi (Jeff) and Joseph Ruggiero (Jaime). She is also survived by relatives and friends.
In lieu of flowers donations may be made in Phyllis' memory to Roswell Park Cancer Institute.
