He was predeceased by: his parents, Lewis Leavens and Gladys Leavens. He is survived by: his wife Elaine Marie Leavens (Stevenson); and his children, Kimberly Marie Leavens (Keith Fetterly), Katrina Marie Eason and Andrew James Leavens (Darlene Leavens). He is also survived by five grandchildren and several great-grandchildren.
