He is survived by: his wife Ann Church; and his children, Gary Church (Michele), Michael Church, David Church (Chrissy), Mark Church (Lynn), Tammy Church, Tina Smith (Wayne), April Ford (Donald) and Melissa Marques. He is also survived by his many grandchildren, great grandchildren, along with many nieces, nephews.
