He was predeceased by: his mother Dorothy Salmon. He is survived by: his father Lee; his brother Greg (Becky); his nephew Brian (Katelyn); his niece Pamela (Ryan); and his friends, Matthew, Denise, Glen, Spiro, Joy, Laura, Nayab and Toni. He is also survived by many aunts, uncles, cousins.

In lieu of flowers, donations can be made to the American Heart Association or Lollipop Farm in David's memory.
