He is survived by: his children, Laurence (Margaret), David Blaho (Jennifer) and Barbara Grosch; his grandchildren, Nicole Triassi (Justin), Justine Barr (Jason), Thomas Blaho (Kasandra), Victoria Mortimer (Corey), David John Blaho and Alyssa Grosch; and his great grandchildren, Landen, Olivia Triassi, Jameson, Jorden, Jaylene, Jocelyn Barr and Esmeralda Mortimer.
In lieu of flowers, memorial contributions may be made to The Dementia Society of America in Jack's name.
