She is survived by: her husband Jack; her children, Kevin Cook (Melissa), Jack Hayes (Saorise), Austyn Hayes (Elizabeth) and Alexis Hayes (Zachary Combs); and her siblings, Daniel Cook, John Witteman and Tracy Robinson (Cook).
