He was predeceased by: his father Robert A. Lookabaugh. He is survived by: his mother Doris Lookabaugh of Pleasant Valley, NY; his close friend Leslie Wilson; his sisters, Krystal Rothaupt (Richard) of Rhinebeck, NY, Holly Lookabaugh-Deur (Sean) of Muskegon, MI and Bobbi Reynoso (Sergio) of Las Vegas, NV; Montana and Beck.

In lieu of flowers, donations may be made in honor of Chad and the sport he loved to the USTA Foundation at ustafoundation.com which supports the National Junior Tennis League.
