He is survived by : his mother Carrie Chadwick (Nash); his brother Jason; his wife Nova Chadwick (Rexroad); his children, Willow Patience Chadwick, Orion Patrick Chadwick and Roman Hayes; his siblings, Heather Nowicki, Christine Baker (Travis), Lisa Flack (Shawn), Kelly, Stephenie, Guy Chadwick, Nathan Chadwick (Angelica), Joshua Shaw (Janna), Paul (Téa) and David (Sarah); his grandparents, Clara Morey and Malcolm Dunn; his siblings, John, Richard and Jackie; and his close friend Nick Johnson. He is also survived by many nieces, nephews and cousins.

Visitation was held on Friday, April 26th 2024 from 10:00 AM to 11:00 AM at the Baker Funeral Home (11 Lafayette Street, Queensbury, NY 12804). A burial was held on Friday, April 26th 2024 at 11:15 AM at the St. Alphonsus Cemetery (52 Luzerne Rd, Queensbury, NY 12804). A graveside service was held on Friday, April 26th 2024 at 11:30 AM at the same location.
