He is survived by: his wife Janice Collins (Thomas); his son Thomas Collins (Kelly); his stepchildren, David Adams (LisaBeth) and Cathy Mahar (Ken); his grandchildren, Thomas "TeeJ" Collins-Gutierrez (Rob Gutierrez-Collins), Sarah Collins (Tara), Antigony Adams-Ryan (Ben Ryan), Myranda Adams, Gabriella Adams, Michael Adams (Megan), Kyle Mahar (Becky), Shannon Mahar (Doug Avery) and Katie Mahar; his great grandchildren, Axle Ryan, Athea Ryan, Arianna Ryan, Atlas Ryan, Aries Adams, Tyler Adams, Cooper Adams, Natalie Adams, McKenzie Adams, Claire Mahar and Evan Mahar; his sister Loraine Mastromarchi (Thomas); and his caregivers, Paul and Adam of Peter's Hospital Emergency Department. He is also survived by several nieces, nephews, great-nieces, and great-nephews.
