He was predeceased by: his wife Joanne [Trannell] Formicola. He is survived by: his children, Maryann Roberts (Jay Witkiewitz), Al Formicola (Katie) and Frankie Formicola (Lisa). He is also survived by 6 grandchildren; 4 great-grandchildren; 4 great-great grandchildren; many relatives and friends.
In lieu of flowers, donations may be made to Western New York State Veterans Home, 220 Richmond Avenue, Batavia, NY 14020 in his memory.
