He was predeceased by: his wife Rose; his parents, Salvatore DeFazio and Josephine DeFazio; his siblings, Leonard, Alphonso DeFazio, Connie Penepent, Rose Butera, Josephine DeGoris and Felicia Boies; his daughter-in-law Barb DeFazio; and his great grandchildren, Melanie Lynn and Kayelynn Briana.
He is survived by: his sons, Sam and Chris (Vicki); his grandchildren, Brian (Kristi), Christopher and Stephanie (Dave); his great grandchildren, Lily Ann, Benjamin Brian, Kaitlynn Christina and Davey Christopher; and his sister Mary Dee Ognibene. He is also survived by many nieces and nephews.
