She was predeceased by: her father Anthony Mazzacua. She is survived by: her mother Theresa Mazzacua; her husband Jim; her children, Michelle Sisneros (Paul) and David Culbertson; her grandchildren, Samuel and Alice; and her siblings, Tracy Gleason, Dr. Andrea Setlik (Robert) and Douglas Mazzacua (Flor). She is also survived by several nieces, nephews, aunts, uncles, and cousins.
Memorial donations in Pam's name may be made to the American Cancer Society.
