She was predeceased by: her parents, Albert Hofmann and Kathryn Hofmann; and her brother Peter. She is survived by: her sons, Shaun and Ross; her siblings, Richard (Gail), Suzanne (Camilo) and Barbara (John); and her former husband Terry. She is also survived by many loving nieces and nephews, several loving cousins, many fabulous friends.

In lieu of flowers, the family asks that donations be made to the American Cancer Society in Carolyn's honor.
