He was predeceased by: his parents, Walker Cook and Carrie Cook (Parks); and his brother-in-law James Gillis. He is survived by: his children, Joshua Cook (Tresa) of Hornell, Gregory Cook (Bryn Baube) of Rochester and Jennifer Fisk (Jody) of Wayland. He is also survived by 2 sisters; many grandchildren; great-grandchildren; nieces, nephews, & cousins.
Josh's family request that in lieu of flowers, memorial contributions in his name be made to a charity of the donor's choice.
