He is survived by: his children, Mark Chotkowski (Laura), Kathy Chotkowski and David Chotkowski (Amy); his sister Antoinette Chotkowski; and his sisters-in-law, Marylou Chotkowski and Barbara Chotkowski. He is also survived by several nieces and nephews.
