He was predeceased by: his father Clyde Underkoffler; his brother David Underkoffler; and his nephew MarcJon Bettleyon. He is survived by: his wife Laura Miller Underkoffler; his children, Susan Schaffer and Michael Underkoffler (Eileen); his grandchildren, Zackery Schaffer, Kody Schaffer, Elizabeth Underkoffler and Jonathan Underkoffler; his great grandchild Koda Schaffer; his brother Philip Underkoffler (Carolyn); his brother-in-law Robert Bettleyon; and his nieces and nephews, Vicki Sponaugle (Kirk), Amber Schultz (Kris), Leslie Gates (Adam), Lara Silvis (Jasper) and Douglas Underkoffler. He is also survived by numerous other nieces and nephews, great-nieces and nephews and great-great-nieces and nephews.
