He is survived by: his wife Barbara Jean Formica; his children, Daniel Formica Jr., Tracy Chase (Scott), Joshua Formica (Donna), Shelley Dostie (Frank) and Tamar McKenzie; his grandchildren, Derek Chapmen, Amber, Jacob, Caliegh Plue, Gabriel Formica, Derrick Jerew, Bethany and Georgianna Dostie; and his siblings, Betty Wibby, Karen Nichols, Rosanne Sapula, Salvatore Formica, Louis Formica, Carmella Barrett (Bob), Dorothy "Dottie" Formica and Marion Page (Bob).
A burial was held on Monday, October 5th 2026 at 1:45 PM. The military honors was held on Monday, October 5th 2026 at 2:00 PM at the Gerlad Bh Solomon Saratoga National Cemetery (200 Duell Rd, Schuylerville, NY 12871).
