He was predeceased by: his parents, Cosmo Azzara and Antoinette Azzara; his wife Charlene; and his granddaughter Brenna. He is survived by: his children, JoAnn, Kathleen, James and Gauge; and his brothers, Richard, Anthony and Cosmo. He is also survived by 4 grandchildren, 3 great grandchildren.
