She was predeceased by: her parents, Louis Kozlowski and Agatha Kozlowski; and her siblings, Florian Kozlowski (late Lillian), Marian Przepiora (late Joseph), Dorothy Hoelzl (late Raymond), Louis Kozlowski Jr. (Dorothy), Dolores Ostrowski (late Alfred), Lorraine Bednarz (late Robert), Richard Kozlowski (Judith), Lucille Reed (late Kenneth) and Patricia Pericozzi (late Louis).
She is survived by: her sons, Kenneth (Kelly) and Robert Czworka; and her grandchildren, Carly, Madison, Brandy, Crystal, Kelly, Alicia and Dennis. She is also survived by many nieces, nephews, cousins, and friends.
Even with her battle with dementia/Alzheimer's—she recognized faces and remembered names.
