He was predeceased by: his parents, Rose DeFazio (Manna) and Sylvester DeFazio. He is survived by: his sons, Anthony (Gina Gentile) and Mark (Courtney Boffa); his child-in-law Courtney; his children, Gina and Frank; his granddaughter Amanda; his great grandchildren, Olivia, Sophia, Elizabeth and Michael; his wife Theresa C. DeFazio; his brother Dr. Victor DeFazio (Sheila); his children, Theresa Lombardo (Frank) and Poppy; and his grandchildren, Olivia Lombardo, Sophia Lombardo, Elizabeth DeFazio and Michael DeFazio. He is also survived by many nieces, nephews and cousins.
He also received the Dr. Norbert Leeseberg Spirit of Volunteerism Award in 2016 and the President's Service Award in 2024 as a volunteer with AmeriCorps Seniors RSVP.
