He was predeceased by: his mother Loretta. He is survived by: his wife Jayne; his daughter Jilian; his father Anthony; and his sister Marie. He is also survived by in-laws, cousins, and dear friends.
In lieu of flowers, memorial donations may be made in Phil's memory to The Glaucoma Foundation or the Broadway League Foundation.
