He is survived by: his wife Bianca (Franchini); his mother Rosemarie Formica (O'Rourke); his mother-in-law Phyllis Franchini; his children, William C. Formica and Juliana Salzarulo; his siblings, Kenneth Formica (Andrea) and Norma Formica Comisar; his grandchildren, Salvatore, William, Giuseppe, Liam, Francesca, Lucia and Valentina; and also Salvatore Salzarulo.
He later received the Alumni Association Award from Wagner College for his philanthropic contributions to the athletic program.
