He was predeceased by: his parents, Albert Kouba and Mary Kouba (Gavan). He is survived by: his children, Bruce Kouba, Jillian Kouba and Matthew Kouba; his stepson Joey Chmielewski; his sisters, A. Dailor (Kouba) and Patricia Arbetman (Kouba); his grandson Rylie; his stepgrandson Andrew; and his former wife Debbie Kouba.
