He was predeceased by: his parents, John Fred and Edith Kowalik Fred (Hulsopple). He is survived by: his siblings, Linda Stevens, John Kowalik, Thomas Kowalik, Robert Kowalik and Michael Kowalik; and his friend Linda Keefe.

“Linda and family: I am very sorry to here this sudden passing of Fred. I have only known Fred for a few years through the frear park golf league. I did have the pleasure of golfing with Fred quite a bit last year and was looking to get together with him again this year. I am very sorry for his loss to the family. Fred was a good friend of mine and seem to be the person who would do anything for anyone. I will miss his friendship immensely. God bless to all of the family and Linda Dan Keefe”
