She was predeceased by: her parents, Lawrence Zizio and Domicela Zizio (Zabinski); her siblings, Frank Zizio, Wilma Mlynarski, Jean Baron and Casimer Zizio; her brother-in-law Edwin Kolek; her parents-in-law, Mary and Stanley Kolek; and her niece Celia Zizio.
She is survived by: her husband Frederick P. Kolek; her children, Cindy Forte (Anthony), Brian Kolek (Barbara), Kevin Kolek (Eileen), David Kolek (Ellen) and Steven Kolek (Cheryl); her grandchildren, Steven Forte (Kim), Bryan Forte Michael (Kim), Daniel Forte (Hannah), Brittany Kolek (Marquis), Brandon Kolek (Adrianna), Jeffrey Kolek, Matthew Kolek (Ashley), Rorie Kolek, Zach Kolek, Rylee Kolek (Ryan), Haleigh Gotham (Brandon) and Lauren Newnan (Ryan); and her great grandchildren, Marcel, Hudson and Hadley Jo. She is also survived by many nieces and nephews.
