He is survived by: his parents, Jose Fuentes and Maximina Rosa; his siblings, Connie Fuentes and Moises Rodriguez; his daughters, Stephanie Ortiz, Jennifer Ortiz, Autumn Ortiz-Miu (Ty), Ashley Ortiz and Dakota Ortiz; his son-in-law John VanSlyke; and his close friend Matthew Zanichelli. He is also survived by several nieces, nephews and grandchildren.
