He was predeceased by: his parents, Jesse Smith and Rena Wayman. He is survived by: his wife Betty Smith; his children, Cindy Stevenson (Scott), Philip Smith, Jr. (Roxanne) and Renne Grossman-Smith (Raymond); his siblings, Larry Smith, Linda Smith and Carol Finch; his grandchildren, Chris, Carly, Nichole, Avery, Morgan, Julie and Kendra; his great grandchildren, Noah, Abby, Mya, Kai, Nova, Sylvia, Hailey, Teegan, Greyson, Bode, Beckett, Aden, Lane, Kellan, Gavin and Meaghen; and his cousin Betty Carlson. He is also survived by several nieces, nephews and cousins.

Memorial contributions in Philips name may be made to Shriners Hospitals for Children 2900 N. Rocky Point Drive Tampa, FL 33607 US, or any local SPCA.
