He is survived by: his parents, William Kozlowski and Kimberly Kozlowski (Votypka); his sister Emma Kozlowski; his grandmothers, Lorraine Votypka and Mary Ann Kozlowski; his aunt Lisa Kozlowski; his uncle Michael Votypka; his friends, Mark, Amy, Mallory and Julia Martin; and his pet Louie.
