He is survived by: his mother Dawn Juckett; his sons, Timothy Baker and James Durham; and his siblings, Diana Perez-Glass, Sharron Henry (Keith), Stephen Baker (Tara), Michael Baker (Renee) and Kelley Baker. He is also survived by his family and friends.
