She was predeceased by: her parents, Malcolm Carr and Cora Carr (Burningham); her brother Kenneth Carr; her nieces, Nicole Versace and Tricia Anson; and her sister-in-law Carlye Carr.

She is survived by: her siblings, Frances Versace (Rochester) and Tom Carr of Hillsborough, NC; her children, Tammy Adams (Tom), Timothy Ward Saratoga (Rosa) and Stephen Ward of Lewisburg, OH.; and her grandchildren, Mandy, Ben, Damaris, Matthew, Justin, Breanna and Nathan.

In lieu of flowers, donations can be made in Phyllis's name to River Hospital in Alexandria Bay.
