He was predeceased by: his parents, David A. Lewis and Lynn Lewis. He is survived by: his wife Ann Marie Lewis (Liuzzo); his son Nicholas Lewis (Jessica); his granddaughter Charlotte; his daughter Jessica Carrick (Chance); and his nieces, Ann Marie Atkins (Rocky), Rachel Atkins (Doug) and Amanda Atkins. He is also survived by his brothers and sisters-in-law and their children.
In lieu of flowers, the family has established a GoFundMe account, and all donations will be directed to the Mayo Clinic for MSA Stem Cell Research in memory of Dr. David J. Lewis. The family has chosen to send donations to Stem Cell Research in memory of Dr. David J. Lewis.
