He was predeceased by: his parents, Ottis and Collingsworth, Sr. (Risner) of Arizona; his grandson Rhilynn Alan Risner; and his siblings, Todd Risner, Brenda Collingsworth, JoAnn Miller, Margaret Olivieri and Judith Tackett.
He is survived by: his children, Chris Collingsworth of Lakeview and Lindsay Collingsworth of Columbus; and his sisters, Linda Hattery, Sue Collingsworth, Carol Nevers, Dee Mears and Carol Thornsbury. He is also survived by three step-children; ten grandchildren.
