He was predeceased by: his father Harry Maddox; his wife Catherine; his mother Catherine Patko Maddox of Norton Ohio; his siblings, Martha Ingersoll, Dorothy Arbogast, Genevieve Gipson, Mary Patko, Katherine Kasper, James Maddox, Harry Maddox, Jr. and Charles Maddox; and his niece Deborah Ingersol.

He is survived by: his daughter Becky Kouns (Shane); and his grandchildren, Caleb Kouns and Sarah Kouns.
