He was predeceased by: his parents, Lionel Cook and Phyllis Corbin Cook; and his siblings, Marybeth Cook, Cindy Cook and Leslie Cook.
He is survived by: his wife Beverly Cook; his former wife Linda Tlusty; his children, Dana Durwin (Jim) and Dale Cook (Shannen); his grandchildren, Austin, Chase, Sam, Jenson and Kaylyn; his niece Audrey Jo Olson; his great-nieces, Alison and Amelia; his stepchildren, Jeffrey Zayda, Cicada Zayda and Jennifer Wagar; and his stepgrandchildren, Alexis, Colleen and Delaney. He is also survived by five great-grandchildren.
Visitation will be held on Saturday, October 25th 2025 from 11:00 AM to 2:00 PM at the Rose Hill Funeral Home (3653 W Market St, Akron, OH 44333). A funeral service will be held on Saturday, October 25th 2025 from 2:00 PM to 3:00 PM at the same location.
