He was predeceased by: his parents, Dewey Middleton and Eva Jean Middleton (Loy); and his brother Jim Middleton. He is survived by: his wife Patricia Middleton; his sons, Chris Middleton (Kristen) and Travis Middleton (Linda); his grandchildren, Chayse Jordan Middleton (Tianna), Cade Christian Middleton, Prudence Rose Middleton and Anwyn Jude Middleton; and his siblings, Sandy Middleton Roose, Teresa Frank (Denny), Dewey "Bud" Middleton (Debbie) and Tim Middleton (Luann). He is also survived by several nieces and nephews.
