She was predeceased by: her parents, Robert Rogers and Anna Mae Rogers (Cranmer); her son Kevin Hoffmann; and her siblings, Herbert, Robert, Ralph, Roland Rogers, Helen Hildreth, Marion Kiger, Jeanette Welsh and Delores McGuire.

She is survived by: her husband William H. Hoffmann; her children, Tracy Webb (Allen), Ken Hoffmann (Mary) and Jay Hoffmann (Shannon); her grandchildren, Alison Feazell-Webb (Jamie), Alex Webb, Alayna Webb, Sarah Hoffmann and Jacob Hoffmann; her great grandchildren, Raelynn Woodward and Sully Bernard; and her niece Sharon Taylor of New Jersey. She is also survived by nieces and nephews.

In lieu of flowers, donations may be made in Phyllis's honor to The Cat House Feline Sanctuary in Ashland.
