He was predeceased by: his parents, Paul Daniel and Myrtle Ruth Cook (Hauck); his son David Cook, II; and his brother Paul "Pete" Cook, II. He is survived by: his sons, Craig Cook (Heidi) and Daniel Cook (Lore); and his brother Albert "Pat" Cook (Sharon). He is also survived by nieces and nephews.
