She was predeceased by: her father Herman Mayberry; her husband Donald Cecil Cook; her son Thomas A. Cook; and her sisters, Pauline Steed and Laura Wherley.
She is survived by: her children, Michael Cook (Debbie) of Barnesville, Nancy Smith (Terry) of Barnesville and David Cook (Jill) of Barnesville; her daughter-in-law Joyce Cook; her grandchildren, Rachel Cook (David Koch), Leah Shirer (Travis), Candi Eccleston (Adam), Nathan Smith (Christy), Matthew Cook (Marta), Mark Cook, Jordan Cook (Lauren) and Allie Cook; her great grandchildren, Lena, Grant, Julia, Caroline, Luke, Gracie, Gavin and Kavin; and her caregivers, Betty Gabel, Delilah Weaver, Mary Carpenter, Karla Thomas, Cindy Messenger and Roberta Duvall. She is also survived by several nieces and nephews.
