He was predeceased by: his parents, Charlestine Booker and Fred Booker Sr.. He is survived by: his children, Clyde Levitt Jr. and Shelby Charlestine Levitt; and his siblings, Addie (James), Fred Jr. (JoAnn), Maurice (Rena), Robin, Shawn (Theopolis II), Kevin (Angyne), Meeko and Duanne (Johanna). He is also survived by nieces and nephews, cousins, aunts, uncles, and dear friends.
