He is survived by: his wife Nancy; his daughters, Elizabeth "Liz" DeGirolamo (John), Christine Erickson (Mike Merritt), Patty Erickson (Shawn Pearson) and Heidi Erickson; his grandchildren, Amalia DeGirolamo, Danny DeGirolamo, Nicholas Erickson, Christopher Erickson, Michael Krolik and Atticus Erickson; and his siblings-in-law, Paula Alexander (late Ronnie) and Gary Walters (Kiyoko).

In lieu of flowers the family suggests memorial contributions be made in Jack's name to a charity of your choice.
